11.        如曾更名，提供更名公证
12.        评估费澳币600（信用卡支付 visa或MasterCard或American Express）

14.        英文的3个项目报告（即3个CE）和1个总结各方面能力的述职报告(Summary Statement of Professional Engineer, 这个大约写400多字)。
15.        做一个清单，总结出自毕业以后，参加过哪些正式的培训或学习？曾经递交过哪些论文？参加过哪些学术会议？哪些短期培训？研讨会？技术会？自学或钻研过哪些专业书籍（包括期刊、学报、手册等）？ 请详细列出时间、地点、主题、内容。（即官方要求的CPD，an account of continuing professional development）
