I just get admission from University of North Carolina at Chapel Hill.The Graduate School asked me to fill out a form of immunization record ASAP or it will delay my registration. On the form, there are a lot of name of vaccine that I should prove that I have taken and some even should be taken 12 months of age. I cannot find such record and also it requres signature of doctor and I wonder who could be reponsible for it? I am very worried about it since the deadline for mailing the form is very close. Please help me!!!!! |
