现在关于填写I765表时有几个问题想询问一下
1. 里面有关曾用名和中间名的空格，没有的话是填N/A还是直接空着比较好呢？
2. 第13项Place of Last Entry into the U.S. 是填写国家（China）还是城市，如果是城市的话是填出境城市（shanghai）还是在美的入境城市（Detroit）呢？
3. 第14项和第15项Manner of Last Entry (Visitor, Student, etc.)和Current Immigration Status (Visitor, Student, etc.)填写的身份应该怎么写呢？我老婆是J2VISA，就直接写J2吗？
