DS-2019表格签字问题

大A您好：我约了下周三面签。想咨询个关于DS-2019表格签字问题。
这是美方机构的说明，如下：
You  should  sign  the DS-2019 in English with  blue  ink on the  bottom  left  corner,  list  your city  and  country  on the  bottom  center  line,  and then date the form on the bottom right line.  
请问：只要手签吧？还需要同时print 吗？blue ink是纯兰还是蓝黑？date写收到表格的日期行吗？
