我去年7月中登陆,到现在还没收到枫叶卡,也没收到过任何关于枫叶卡的任何信件.(同去的一位朋友9月份收过一份通知到移民部面试,是相片质量问题,后来11月中收到枫叶卡).我在CIC网站上看到以下说明,我现在国内,请教版主,哪里可以下载IMM5451表格?谢谢!!!
Solemn Declaration Concerning a
Lost, Stolen, Destroyed or Never-Received
Permanent Resident Card (PR Card)
This form is for you if you are a permanent resident who:
.Provided your Canadian mailing address to CIC at the point of entry into Canada and have not received your Permanent Resident Card after at least four weeks.
or
.Provided your Canadian mailing address to CIC sometime after entering Canada and have not received your Permanent Resident Card after at least four weeks. If you are a permanent resident, you have a maximum of 180 days to provide your Canadian address after entering the country.
or
.Possessed a valid Permanent Resident Card that was lost, stolen or destroyed but you do not wish to reapply for a new card immediately. Please note that applications for existing permanent residents will be available after September 15, 2002. You can obtain a PR Card application kit (when available) from this site, or by contacting the PR Card Call Centre at 1-800-255-4541 after this date.
As of December 31, 2003 all Canadian permanent residents will require a PR Card as proof of status when travelling outside Canada.
